Clinical Study of Gastric Cancer


Abstract
The cases of 144 patients received operations at the dept. of surgery, Kang Nain GeneraL Hospital between 1st, Sep. 1976 and 31st, Aug. 1983 with gastric cancer were studied statistically and following results were obtained.
1) Sex ratio was 1.8:1 male to female, and most frequent age group was from 50 to 59 years.
2) The most common symtoma were epigaatric pain 66.6%, epigastric tenderness 68.0%, weight loss 44.4%, palpble mass in the epigastric area 47.2% and nausea and vomiting 38.8%.
3) Duration of symtoms to operation were predominate in the group of 3 to 6 months with 39.4%, less than 3 mouths with 18.0% and in the group of more than 1 year with 26.3%.
4) The patient with blood group A were 41.6%, and blood group B, AB. & 0 were 36.1%, 13.1% and 9.0% respectively.
5) The radiological findings of upper G?I series were seen filling defect in 53.0%, filling defect with obstructin in 21.4%, filling defect with ulcer in 16.3% and Ulcer in 9.1%.
6) On the location of the tumors, tumors were found 61.1% in the antrum, 18.7% in the body and fu.ndua, 6.2% in the cardia to the lesser curvature and 10.4% in the prepyloric and body to the great curvature.
7) Incidence of the gastric cancer with regional lymphnode metastasis and celiac node metastasis were 79.8% and 36.1%. Distant metastasis found during operation were as followings: Omenturn 19.4%, liver 18.0 %, transverse colon 17.3%, and biliary system 9.7%.
8) Five cases were diagnosed as early cancer by endoscopic biopsy. 1 Out of 50 cases with endoscopic biop8y was adenocarciaoma in situ.
9) Indications for surgical operations in 122 cases were proven diagnosia of gastric cancer 84.7%, plyoric obstruction 33.3%, gastric hemorrhage 8.3%, gastric perforation 6.9% and clinically bengn lesion 5.5%.
10) Of the 94 patients operated under the diagnosis of gastric cancer, following surgical procedures were performel: Total gasrectomy 8.3%, subtotal gastrectomy 51.3%, explora
tory laparotomy and biopsy 16.6%, and by pass procedure 19.4%. The resectability was 59.7%.
11) Post operative complications were as follows: Infection 17 cases, (48.5%), obstruction 5 cases(14.2%), peritonitis 5 cases (14.2%), bleeding 4 cases(11.4%) and pneumonia 4 cases (11.4%).
12) Operative morality rate was 6.2%.
13) Pathologically. 90.0% of the tumors disclosed adenocarcinoma.
